Cash-Pay and No-Insurance Disclosure

Hayat is a cash-pay service
We do not bill, accept, or coordinate medical insurance for the services we provide. We are not enrolled with commercial insurance plans, and we do not submit claims for visits, consultations, or management services. Your membership is not health insurance.
How you pay
All services, including membership, consultations, prescribed therapies, and laboratory testing where included, are paid through a single monthly membership fee, or per-item prices where offered, charged to the card you keep on file. Laboratory testing included in your plan is arranged and paid by us; you will not receive a separate lab bill and your insurance will not be involved.
Your pharmacy benefit is separate
Your prescription drug benefit is independent of Hayat. If you are prescribed a brand-name medication, the pharmacy that fills it may bill your prescription drug plan, and any coverage or copay is determined by your insurer and the pharmacy, not by us. Where useful, your Hayat clinician will prepare prior authorization requests and letters of medical necessity to support pharmacy coverage. This is clinical support, not insurance billing, and coverage decisions rest with your insurer.
Superbill policy
On request, we will provide a superbill, which is an itemized receipt showing the services you received and the amounts you paid. You may submit a superbill to a health savings account, a flexible spending account, or your insurer on your own. We prepare superbills as a courtesy. We do not submit them for you, we do not file insurance claims on your behalf, and we cannot guarantee that any plan will reimburse you. Eligibility for HSA or FSA reimbursement varies by plan; please confirm with your plan administrator before relying on it. Superbill requests may be made at iaijazuddin@signal-therapies.com and are fulfilled within 10 business days.
Acknowledgment
I understand that Hayat will not bill my medical insurance for its services, that my membership is not insurance, and that any superbill is provided for my own use without any guarantee of reimbursement.
